One island, two hours across, two separate service areas

Hawaiʻi Island is larger than every other Hawaiian island combined. Hilo and Kailua-Kona sit on opposite coasts, roughly two hours apart by road, and a service that exists on one side is not necessarily available on the other. Patients are routinely told the appointment is across the island.

For residents of Kaʻū, Puna, Hāmākua, or the Waimea saddle, that trip can consume most of a day — before accounting for weather on Saddle Road or vog on the leeward side. When ongoing medication management depends on showing up every few months, a half-day round trip is the thing that quietly ends treatment. Video care is offered island-wide for that reason.

Care includes adult ADHD evaluation, medication management, anxiety and depression, bipolar disorder, and diagnostic clarification and second opinions for adults whose treatment has stalled.

Who you will see

You will work directly with Dr. Reginald Casilang, DNP, PMHNP-BC, FNP-BC, a dual board-certified psychiatric and family nurse practitioner, for your evaluation and ongoing care.

Care that does not depend on which side you live on

Hawaiʻi Island patients often describe being routed back and forth: an intake in Hilo, a follow-up offered only in Kona, a prescriber who leaves and a replacement who is based on the other coast. Each handoff restarts the history, and the treatment plan resets with it.

Working with one provider by video keeps that history intact. The evaluation, the medication decisions, and the reasoning behind them stay in one place, regardless of where on the island you happen to be living or working.

I live in Kona but was referred to Hilo. Does that matter here?

No. Care is delivered by secure video, so which side of the island you live on does not affect availability, scheduling, or continuity of follow-up.

I have seen several prescribers and keep starting over. Can that be sorted out?

Yes — this is a common reason adults seek diagnostic clarification. Prior records and treatment history are reviewed so the plan builds on what has already been tried rather than repeating it.

Do you prescribe for patients in rural parts of the island?

Yes. Prescriptions are sent electronically to the pharmacy you choose, including in Kaʻū, Puna, Hāmākua, and other rural areas.
